REGISTRATION FORM FOR PRIVATE MUSIC LESSONS

All applied music students, pre-registered or not, are required to either fill out this form or fill out schedule cards in the Music Office, Dixon Hall, Room 100, during registration week. The deadline for the fall semester, 2004, is Friday, August 27. Admission to the course is not guaranteed beyond that date. Filling out this form does not guarantee admission to the program.
